I am hereby making an advance donation for participation in religious services. I understand and acknowledge that all past, present or future donations I have made and will make to the Church are property of the Church to use at its discretion for its religious and charitable purposes. I understand that donations to the Church are generally not refundable and I am bound by the policies of the Claims Verification Board.
